The OSEA release forms may be downloaded, printed, and submitted final program payment.

Release forms must be submitted with the final payment prior to the initiation of a program.

Persons who do not submit all complete forms prior to departure understand that they will be prohibited from participation in the program and that the costs that this may incur will be the sole responsibility of the person who has not submitted all necessary forms to allow for their participation.
